Dandruff or Dry Scalp? How to Tell the Difference
Flaking is not one condition. It is two, and they need opposite care.
Part your hair and there they are — pale specks on your shoulders, or yellowish patches stuck close to the skin. Most people respond the same way to both: wash more often, scrub harder. For one kind that makes it worse; for the other it never reaches the problem. This guide sets the two side by side, explains what the scalp is actually doing, and covers what to change at home — and the signs that mean a doctor, not a different shampoo.

You part your hair and there they are. But flaking comes in two kinds, and they need opposite care — get it the wrong way round and it gets worse.
Two kinds of flaking

Most people respond to flakes by washing more often and scrubbing harder. For dry flaking that pours fuel on the fire; for oily flaking it still does not clear what is actually there. The first step is not a new product. It is working out which one you have.
Why skin sheds early: the barrier
The outermost layer of the scalp, the stratum corneum, is a barrier. In healthy skin its cells sit tightly packed and are held together by a lipid matrix — ceramides, cholesterol, fatty acids — like bricks and mortar. That layer holds water in and keeps irritants out.
When the lipids run short, gaps open between the bricks. Water leaves faster and irritants get deeper. The skin's response is to speed up turnover. A cell normally takes close to a month to travel from the base of the epidermis to the surface and shed; with a damaged barrier it takes far less, and immature cells come away in visible clumps. What you see as flakes are those cells, shed early.
So shedding does not automatically mean dryness. A disordered barrier flakes on an oily scalp too — which is why moisturising alone, or cleansing alone, so often fails.
Malassezia: it lives on sebum
Malassezia is a yeast that lives on everyone's scalp. It is not a sign of poor hygiene. It produces no oil of its own; it lives by breaking down the lipids in sebum, and that process releases free fatty acids, mainly oleic acid. Some people's scalps are particularly reactive to those fatty acids, and that is where the redness and shedding come from.
Two things are worth being precise about.
First, carrying Malassezia does not mean you will flake. Individual sensitivity is what decides it — which is why things get suddenly worse during stress, poor sleep, or after an illness.
Second, adults and infants are not the same case. In adults the link between Malassezia and scalp shedding is well established. In infant cradle cap, the dermatology literature says the yeast is implicated but that its exact role is unclear; the driver named first is sebaceous activity responding to residual maternal hormones. The thread connecting them is not the organism. It is sebum. Both appear where the skin makes the most oil, at the two points in life when oil output is highest — the first months, and puberty onward.
Why Hong Kong makes it harder
- Year-round humidity. Above 70% is normal. The scalp stays occluded, and hats, helmets and tied-up hair make it worse.
- Crossing into air conditioning. From 30°C and 80% humidity outside to 22°C and dry air indoors, several times a day. Each crossing is a humidity cliff.
- Sweat left to dry. Salt stays on the scalp after sweat evaporates, and with friction on top of it the itch intensifies.
- Shampoo that is too strong. Many people equate "clean" with "tight". Stripping too much lipid weakens the barrier, and oil rebounds faster.
Dry flaking: what to change
- Lower the water temperature. Hot water strips scalp lipids faster.
- Use fingertips in small circles, never nails.
- Lather shampoo in your hands rather than pouring it straight onto the scalp.
- Moisturise while the scalp is still slightly damp, not after it has dried out.
- In long air-conditioned days, add humidity or move out of the direct airflow.
Oily flaking: what to change
- Wash more often if you need to, but not harder. Frequency and pressure are separate things.
- Wash the scalp, not the lengths. Let the lather sit for a moment before rinsing.
- Rinse thoroughly, and keep conditioner on the mid-lengths and ends rather than the scalp.
- Do not scrape flakes off with your nails. Removing the surface does not settle what is underneath, and it breaks the skin.
- Free the scalp soon after a hat or helmet rather than leaving it covered for hours.
When to see a doctor
Some things are not a shampoo problem. See a doctor if you have:
- Thick, sharply bordered patches, or patches spreading past the hairline
- Itch that keeps you awake, or scratching that draws blood or leaves yellow crusting
- Hair loss in the same spot, or an area of loss that is growing
- A child whose scalp problem has lasted beyond twelve months
- No improvement after four to six weeks of adjusting your routine

Children, pregnancy and G6PD
Children. Infant cradle cap and adult shedding are different situations. Cradle cap is usually not itchy, does not affect health, and resolves over months; adult methods — and scraping in particular — are the wrong approach. See the cradle cap guide.
Pregnancy. Hormonal change alters sebum output, and a scalp can turn oily or dry quite suddenly. Ask a doctor or pharmacist before using any scalp product during pregnancy.

Frequently Asked Questions
How do I tell dry flakes from oily flakes?
Look at them and feel the scalp. Dry flakes are small, loose and powdery, they fall onto your shoulders, and the scalp feels tight. Oily flakes are larger, yellowish and slightly greasy, they cling to the scalp, and the scalp feels oily to the touch. If you have both, it is usually a damaged barrier plus sebum — start by adjusting how hard you wash rather than reaching for the strongest product.
Do flakes mean my hair is not clean?
No. Flakes are skin cells shedding early, which has no direct relationship to dirt. Washing too often or too hard strips more lipid from the scalp, weakens the barrier further, and usually produces more flaking, not less.
Will washing more often make the shedding worse?
It depends which kind you have. Oily flaking usually improves with more frequent washing, as long as you are washing the scalp rather than the lengths and using light pressure. Dry flaking gets worse with frequent washing — reduce the frequency, lower the water temperature, and moisturise while the scalp is still slightly damp.
Can flaking spread to the rest of my family?
No. The Malassezia yeast on the scalp is a normal resident on everyone's skin, not something passed between people. When several people in a household have it at once, it is usually shared environment and habits rather than transmission.
My scalp itches but I cannot see any flakes. What is that?
Itch does not always come with visible shedding. It can be an early stage of barrier damage, shampoo residue, irritation after colouring or perming, or simply sweat and friction. If it keeps you awake, if scratching draws blood, or if it lasts beyond four to six weeks, see a doctor.
How long before a change in routine shows?
The scalp turns over its outer layer in roughly a month, so four to six weeks is the honest observation window. Do not expect visible change in the first few days, and do not switch products weekly because nothing happened immediately.
